Mamas Milk Club - Consent Form

I understand that my attendance and participation at Mama's Milk Club with The Breastfeeding Center of Orange County is meant to provide me with information and education. Participation in this group does not create a clinical relationship between me and The Breastfeeding Center, LLC or the person leading the group.

I understand that I am responsible to consult with my primary care provider and/or my baby’s primary care provider before implementing any recommendations made during Mamas Milk Club.

I understand photos and/or recordings may be taken by The Breastfeeding Center of Orange County or by other participants in the group at any time while you are participating in in-person or zoom Mamas Milk Club sessions.

I give permission to The Breastfeeding Center, LLC. and the leading IBCLC or other professional to examine me and my baby. I may withdraw this consent at any time.

I recognize that Mamas Milk Club is lead by an IBCLC (Board Certified Lactation Consultant), CBS (Certified Breastfeeding Specialist), CLEC (Certified Lactation Educator) other professionals, however we are not doctors and cannot provide medical advice or supervision in place of my and my baby's primary care physician.

I understand that Mama's Milk Club and its included resources are for general informational and educational purposes only. It does not replace a comprehensive lactation consultation nor does it create a clinical relationship between myself and the IBCLC leading the group. The support provided in Mamas Milk Club does not replace the clinical care of your doctor, lactation consultant, or other health care providers. No information received should be construed as a clinical recommendation for your specific case.

Refund & Cancellation Policy

We’re confident you’ll love the support and community inside Mama’s Milk Club. However, we understand that plans change.

You may cancel your subscription at any time.
Once canceled, you won’t be charged for any future payments.

Please note: We do not offer refunds for past payments.
Access is granted immediately upon purchase, so all sales are final.
